Medical Hair Loss Treatments

Medication is an important option for many people with pattern hair loss. At The Wright Practice, we match treatment to the diagnosis, your health and your preferences, with a discussion of expected benefits, side effects and ongoing care.

Minoxidil

Topical minoxidil is applied to the scalp as a foam or solution. It can help slow pattern hair loss and encourage growth in some men and women. The formulation and instructions depend on the individual and product. Scalp irritation and unwanted facial hair can occur, and temporary increased shedding is possible when treatment begins.

Low-dose oral minoxidil may be considered for selected patients. Its use for hair loss is off-label, meaning it is prescribed outside its licensed indication. It can cause unwanted body or facial hair, ankle swelling, dizziness and palpitations. Rarely, more serious cardiac complications can occur. Blood pressure, cardiovascular history and other medicines need to be reviewed before prescribing, with monitoring tailored to the patient.

Finasteride

Oral finasteride reduces DHT and is a licensed option for male pattern hair loss in eligible men. It can slow further loss and may improve growth, although the degree of response varies.

Potential adverse effects include reduced libido, erectile or ejaculatory difficulties, breast symptoms and mood changes. Sexual symptoms have been reported to persist after stopping. Finasteride is associated with depression and suicidal thoughts. Patients taking finasteride 1 mg for hair loss should stop it and contact their doctor promptly if depression or suicidal thoughts develop.

Topical finasteride may be considered in selected patients. Depending on the formulation, it may be supplied as an unlicensed preparation. Applying it to the scalp does not eliminate absorption or the possibility of systemic side effects. We explain how to use the prescribed preparation safely and what to look out for during treatment.

Dutasteride

Dutasteride suppresses DHT through a broader enzyme-blocking action than finasteride. Oral use for hair loss is off-label in the UK and may be considered in selected patients after reviewing other options.

Possible adverse effects include sexual dysfunction and breast symptoms. Mood changes and the updated psychiatric safety warnings also need discussion. Dutasteride remains in the body for a prolonged period after stopping. Pregnancy-related precautions and fertility plans are relevant to treatment decisions.

Choosing and combining treatments

Treatment choice takes account of the pattern and duration of hair loss, previous treatment, your medical history and how practical a regimen will be to maintain. Some patients use minoxidil alongside a DHT-blocking medicine because the treatments work differently. Combination treatment can add cost and side effects and is not necessary for everyone.

In women, minoxidil is commonly considered first. Selected patients may benefit from specialist-led anti-androgen treatment, such as spironolactone, after appropriate assessment. This is off-label for hair loss and requires individual advice about pregnancy prevention, side effects and any blood monitoring. Finasteride and dutasteride are not routine treatments for women.

Optional genetic testing can be discussed as part of treatment planning; see Genetic Testing for Treatment Selection on our Hair Loss Assessment & Treatment page.

Safety and suitability

Before prescribing, we review current medicines, blood pressure where relevant, cardiovascular conditions, mood history, sexual symptoms and fertility or pregnancy plans. Tell your clinician about any previous adverse reaction to a hair-loss treatment.

Hair-loss medicines are generally avoided during pregnancy and breastfeeding. Finasteride and dutasteride require specific precautions to prevent exposure during pregnancy; topical preparations can also transfer through skin contact. Follow the instructions for the exact product supplied.

Some treatments are prescribed off-label or supplied as unlicensed preparations. We explain what this means and why a treatment may suit you. Follow-up provides an opportunity to report side effects and adjust treatment

Results and ongoing treatment

Early changes may take several months. A meaningful assessment of response often needs around 6 to 12 months, depending on the medicine and diagnosis. Some people experience less shedding or stabilisation rather than visible regrowth, and some do not respond.

Benefits from treatment for pattern hair loss usually depend on continued use. If treatment stops, the underlying process can resume and any gains may gradually be lost. The aim is to preserve existing hair and improve growth where follicles remain active.s
