Testosterone and Male Hormonal Health

Testosterone levels in men decline gradually from the mid-thirties onwards. The symptoms that follow, fatigue, low libido, reduced muscle mass, low mood, poor concentration and weight gain around the abdomen, are among the most frequently missed or misattributed conditions in men’s health. They are not an inevitable consequence of ageing; in many cases they reflect a clinically treatable hormonal deficiency.

Assessment begins with a blood test measuring total testosterone, free testosterone and SHBG (sex hormone binding globulin), interpreted alongside the full clinical picture. Where treatment is appropriate, options include daily topical testosterone gel (Testogel), long-acting quarterly injection (Nebido) or more frequent injections using Sustanon, which can also be microdosed. Monitoring during treatment, including regular blood tests for haematocrit, PSA and free testosterone, is an essential part of safe, effective therapy.

Sexual Health and Function in Men

Sexual health concerns are among the most common reasons men seek private medical advice and among the least likely to be raised with an NHS GP. Erectile dysfunction affects a significant proportion of men at some point in their lives and is frequently associated with underlying cardiovascular risk, hormonal factors or psychological causes, all of which are clinically assessable. Low libido in men is often multi-factorial, with testosterone deficiency, stress, sleep disruption and relationship factors all playing potential roles. Premature ejaculation and sexual performance anxiety are both highly treatable when properly assessed.

Hair Loss in Men

Androgenetic alopecia, or hereditary hair loss, is driven by DHT (dihydrotestosterone) binding to genetically susceptible follicles and causing them to miniaturise over time. It affects the majority of men to some degree by their fifties and often begins earlier than expected. Treatment is most effective when started early, before significant follicle miniaturisation has occurred.

Dr Dan Wright offers dutasteride mesotherapy at The Wright Practice: a targeted treatment that delivers a dutasteride solution directly into the scalp at the follicle level, blocking DHT locally with a substantially lower systemic side effect risk than oral treatments. Dutasteride blocks both subtypes of the 5-alpha-reductase enzyme, producing a more powerful reduction in scalp DHT (around 90%) than finasteride (around 70%), which blocks only one subtype. A full hair loss assessment includes blood testing to identify any treatable underlying contributing factors, including thyroid function, iron status and hormonal markers.

Weight Management and Metabolic Health

Weight gain, particularly around the abdomen, is associated with declining testosterone, insulin resistance, elevated cardiovascular risk and a range of symptoms including fatigue, poor sleep and reduced exercise capacity. A holistic approach to weight management addresses the hormonal, metabolic and lifestyle factors involved rather than treating weight as an isolated problem.

Wegovy (semaglutide) and Mounjaro (tirzepatide) have transformed the medical treatment of persistent weight gain. Both work by mimicking natural gut hormones that regulate appetite, slowing gastric emptying and reducing the background food noise that makes sustained calorie reduction difficult. The Wright Practice offers a supervised weight management programme including an initial consultation, injection technique instruction, personalised nutritional guidance, quarterly clinical reviews and ongoing support. Prostate Health and Urology

Prostate cancer is one of the most common cancers in men and also one of the most treatable when identified early. Prostate MRI is now the recommended first-line investigation for detection, providing significantly more accurate assessment of clinically significant cancer than PSA testing alone. Lower urinary tract symptoms, including urinary frequency, urgency and poor stream, are common in men over 50 and often reflect benign prostatic hyperplasia rather than cancer, but they warrant proper assessment. The Wright Practice can assess urological symptoms, arrange appropriate investigations and refer to a specialist where needed.

Cardiovascular and Preventative Health for Men

Men are at higher risk of cardiovascular disease and tend to access preventative healthcare less frequently than women. A structured cardiovascular risk assessment, including advanced biomarker testing (ApoB, Lp(a), hs-CRP), blood pressure assessment, and where appropriate a CT coronary angiogram, can identify significant risk factors well before a cardiac event occurs.
