Most sexual health concerns can be diagnosed and treated using well-established pathways. Occasionally, however, symptoms persist despite repeated investigations, test results appear contradictory, treatments fail to resolve the problem or patients are left without a clear explanation for what they are experiencing.

These situations can be frustrating, confusing and often anxiety-provoking. Many patients arrive having undergone multiple consultations, numerous tests and several courses of treatment, yet remain uncertain about their diagnosis or the most appropriate next steps.

Understanding The Limitations Of Testing

Modern sexual health testing is extremely powerful, but no test is perfect and no investigation can answer every clinical question.

Highly sensitive molecular tests can sometimes identify organisms that are present but not causing disease. Equally, negative results do not always explain why symptoms are occurring.

Understanding the strengths and limitations of investigations is therefore an important part of reaching a meaningful diagnosis and avoiding unnecessary treatment.

A Broader Perspective

Complex sexual health symptoms often sit at the intersection of several medical disciplines, including sexual health, urology, gynaecology, dermatology, pain medicine and psychological wellbeing.

For this reason, a broader perspective is sometimes required. Conditions such as pelvic floor dysfunction, chronic pain syndromes, dermatological disorders, hormonal factors and anxiety-related symptom amplification may all contribute to symptoms that initially appear infectious in nature.

Stepping back and reviewing the wider clinical picture can often be more helpful than continuing to focus on a single test result or diagnosis.

Our Approach

Our approach begins with listening carefully to the patient’s story, reviewing previous investigations and understanding what has already been considered, excluded and treated.

Where appropriate, we may recommend additional investigations. Equally, there are situations where the most valuable intervention is providing a clear explanation, identifying patterns that may previously have been overlooked and helping patients move forward with a coherent management plan.

The aim is not simply to order more tests, but to provide clarity, context and evidence-based guidance in situations where answers have proved difficult to find. For many patients, this process can be just as valuable as reaching a new diagnosis.
